Ultrasound Features: - Both corpus cavernosa & corpus spongiosum appear uniform. - Both cavernosal arteries are normal in diameter. - Post-papaverine state shows increased systolic flow in cavernosal arteries. - Persistent increase in diastolic flow indicating venous leakage. Remember: - This may be due to an excessive venous leak from the corpora cavernosa. - The exact cause is not known. - It is assumed that tunica albuginea stretches beyond the normal limit and thus cannot obliterate the emissary veins draining the sinusoids during the erection. - Due to the inadequate compression of emissary veins the venous outflow is continuous & erection is never fully obtained. - A venous leak is suspected when persistent high diastolic flow is seen in the cavernosal arteries & the diastolic flow is more than 5 cm/sec.
